Video Summary
The speaker explains a guideline-based approach to diagnosing AV node–dependent PSVT, mainly AVNRT and AVRT/WPW. Diagnosis relies on both clinical history and ECG documentation, because symptoms alone cannot confirm or exclude the arrhythmia. Typical clues include sudden onset, very rapid and regular palpitations, often with neck pounding, and episodes that usually last longer than five minutes. Vagal maneuvers may help but failure to terminate an episode does not rule out PSVT. AVNRT is more common in women and often begins in the 30s, while AVRT often starts in childhood or the teens. ECG during an episode may show narrow-complex tachycardia, but wide-complex presentations can occur and should be treated cautiously as possible VT. Diagnostic tools include Holter monitors, event patches, implantable monitors, and increasingly consumer wearable ECG devices, which improve symptom-rhythm correlation and help catch intermittent episodes.
