The authors report the first case of a patient with a durable left ventricular assist device admitted with cardiogenic shock and managed with biventricular Impella (Abiomed Inc., Danvers, Massachusetts) support as a successful bridge to heart transplantation.

After reading this article the reader should be able to:

  • Identify indications and contraindications for temporary mechanical circulatory support.
  • Identify and discuss common complications and approaches to management of the Impella (Abiomed Inc., Danvers, Massachusetts) device.
  • Recognize the clinical presentation of complications of left ventricular assist devices.
  • Discuss approaches to management of right ventricular failure in the setting of a left ventricular assist device.
