Iatrogenic Left Internal Mammary Artery Perforation Treated With a Covered Stent Via Transradial Approach (JACC Case Reports December 2019)
Inadvertent perforation of the left internal mammary artery during a blind approach to the subclavian vein for pacemaker or central venous catheter insertion is an emergency that requires immediate treatment. Covered stent deployment is a quick and effective treatment, especially in patients with hemodynamic instability. The procedure may be safely performed by using the radial approach.

Learner Objectives

After reading this article the reader should be able to:

  • Recognize unusual complications from pacemaker (PM) or implantable cardioverter-defibrillator (ICD) implantation.
  • Identify left internal mammary artery (LIMA) injury or perforation during PM or ICD implantation.
  • Manage safely and quickly an unstable clinical condition caused by inadvertent LIMA perforation through a percutaneous radial approach with covered stents.
