This review will focus on new insights into coronary plaque progression and destabilization that OCT has provided over the past several years, with a focus on the role of local hemodynamics and endothelial shear stress (ESS), the layered plaque (signature of previous sub-clinical plaque destabilization and healing) and the calcified culprit plaque.
