After reading this article the reader should be able to:

• Identify high-risk patients with adult congenital heart disease (ACHD) and advanced heart failure (HF).
• Select patients for referral to HF and ACHD specialists to consider mechanical circulatory support and heart transplant.
• Compare baseline characteristics of patients with and without ACHD undergoing heart transplant.
• Discuss short- and long-term outcomes of patients with ACHD undergoing advanced HF therapies.
