Radiation-associated cardiac disease (RACD) is a heterogeneous disease carrying a high morbidity and mortality burden, manifesting decades following prior chest radiation exposure. Coordinated management by an experienced team of providers is strongly advocated. Timing of surgical intervention must be individualized, based on the complexity of the disease, comorbidities, and technical difficulty. Percutaneous options are increasingly available, although their use and suitability in RACD is evolving. Multimodality imaging-based screening protocols are crucial to adequately identify those at risk and plan therapeutic procedures. The future should involve developing radiotherapy treatment protocols that minimize the chances of developing RACD.
