Aortic dissection is part of the family of acute aortic syndromes, the most common life-threatening aortic pathology. Early recognition with a high index of suspicion, correct clinical evaluation, and correlation with biomarkers and imaging studies are the cornerstone of the diagnostic algorithm. Early treatment includes effective control of blood pressure and heart rate, avoiding dissection expansion, and end-organ malperfusion. Surgical treatment of Stanford A aortic dissection remains the strategy of choice; on the other hand, medical and endovascular therapy is indicated in patients with a compromise of the descending aorta in its thoracic and abdominal segments. The recognition of risk factors that complicate this clinical condition must be disseminated, and the indication of endovascular therapy at the appropriate time is essential for the success of the intervention.
