Welcome to HF SAP! HF SAP is ACC’s flagship self-assessment program covering the entire field of Advanced Heart Failure and Transplant Cardiology. It's a great resource for:

• Perfecting your knowledge of heart failure - Use HF SAP to identify your knowledge gaps, reinforce your existing knowledge, and learn new information.
• Earning CME/MOC/ECME credit. HF SAP currently offers 25 CME/MOC credits and medical knowledge MOC points. Additional credits will be added in 2021 as more questions are added to the product.
• Completing Your MOC Assessment Option. HF SAP offers the Collaborative Maintenance Pathway (CMP), a new option for cardiologists wishing to meet their MOC assessment requirement. The CMP integrates lifelong learning with assessment and is the shortest path between learning and maintaining your credentials. Learn more about the CMP.

Attention CardioSource Plus Users Planning to Participate in the CMP:

• Your institution's subscription to CardioSource Plus does not include the CMP.  If you want to participate in the CMP, you will need to purchase your own personal version of HF SAP.
• As long as you use the same ACC.org account to access HF SAP through CardioSource Plus and to access your personal subscription to HF SAP, your progress and usage data will be stored in one central location. This includes any time that you have already spent toward your 7 hours of Learning Engagement.
• You have two options for ordering HF SAP:
  
  • Order HF SAP Online through the CMP Enrollment process:
    • Visit ACC.org/CMPEnroll and click on the Enroll in the CMP for Heart Failure button. You will be prompted to purchase HF SAP as part of the enrollment process.
    • Note: technical issues will prevent you from purchasing HF SAP outside of enrollment.
