This study examined how coronary artery calcium (CAC) helps guide statin decisions under the 2026 ACC/AHA dyslipidemia guideline, which uses the PREVENT calculator for 10-year ASCVD risk estimation. Using data from 5,698 participants in the multiethnic MESA cohort, researchers grouped people into statin not recommended, considered, or recommended categories, and also into PREVENT risk bands.<br /><br />Key findings:<br />- CAC strongly separated event risk within each statin eligibility group.<br />- Among people in whom statins were not recommended or only considered, CAC presence identified higher risk, and CAC 0 often corresponded to lower observed event rates.<br />- CAC was most useful in the borderline-risk group (PREVENT 3% to 5%): if CAC was absent, observed event rates fell below the threshold for statin initiation.<br />- However, in people who already met guideline thresholds for statins, event rates remained elevated even when CAC was 0, suggesting CAC 0 should generally not be used to withhold therapy.<br />- Adding CAC improved risk classification beyond PREVENT alone.<br /><br />Observed ASCVD event rates per 1,000 person-years ranged from:<br />- 1.2 with CAC 0 vs 4.5 with CAC present in the “not recommended” group<br />- 2.7 vs 5.2 in the “considered” group<br />- 5.8 vs 16.6 in the “recommended” group<br /><br />Overall, the study supports a guideline-centered role for CAC in the PREVENT era: mainly to refine decisions when risk is near treatment thresholds, especially borderline risk, rather than to override statin recommendations in higher-risk patients.
