Article: Relationship Between Ambulatory Pulmonary Artery Pressures and Heart Failure Hospitalizations in Patients With Chronic Heart Failure

This retrospective analysis pooled data from four CardioMEMS studies to assess whether ambulatory pulmonary artery pressures (PAPs) predict heart failure hospitalization (HFH). The analysis included 4,019 patients with chronic heart failure, followed for up to two years; 3,660 had paired pressure measurements at baseline and six months.<br /><br />Higher baseline systolic, diastolic, and mean PAPs were independently associated with greater HFH risk. For each 1-mm Hg increase, estimated risk rose by about 2% to 4%. Pressure changes over the first six months also predicted subsequent risk: a 2-mm Hg decrease in diastolic PAP was associated with a 16.1% lower risk than little or no change, while a 2-mm Hg increase was associated with a 50.1% higher risk. Similar patterns were observed for systolic and mean PAPs.<br /><br />These relationships were directionally consistent in patients with reduced and preserved ejection fraction. However, at the same pressure, patients with reduced ejection fraction had a higher probability of hospitalization. All three PAP measures showed comparable predictive value.<br /><br />The findings suggest that both current pressure levels and their change over time provide prognostic information and may help guide heart failure care. Important limitations include the retrospective design, differences among the contributing studies, and assessment of time to first hospitalization rather than recurrent events. The study was funded by Abbott, and the results establish associations rather than proving that pressure changes themselves cause changes in hospitalization risk.
