This study evaluated the new high-sensitivity cardiac troponin T generation 6 (hs-cTnT-gen6) assay in patients presenting to emergency departments with suspected myocardial infarction (MI), and compared it with the established gen5 assay. In 3,346 patients, MI was ultimately diagnosed in 18.4%.<br /><br />Key findings:<br />- hs-cTnT-gen6 showed excellent diagnostic accuracy for NSTEMI, essentially matching gen5 (AUC 0.927 vs 0.931).<br />- Fewer patients were classified as having myocardial injury with gen6 than gen5 (35.9% vs 43.5%), likely reflecting assay restandardization and improved analytical characteristics.<br />- A single gen6 cutoff of 6 ng/L ruled out NSTEMI in 30% of patients with 100% sensitivity.<br />- Assay-specific cutoffs for ESC accelerated pathways performed very well:<br /> - 0/1-hour algorithm: ruled out 56.3% with 99.7% sensitivity; ruled in 20.0% with 93.4% specificity.<br /> - 0/2-hour algorithm: ruled out 51.1% with 99.7% sensitivity; ruled in 23.9% with 92.7% specificity.<br />- Results were consistent across subgroups and were confirmed in an external validation cohort.<br />- Prognostic performance for 5-year all-cause mortality was also high and similar to gen5.<br /><br />Overall, hs-cTnT-gen6 is clinically comparable to gen5 but with better analytical sensitivity, fewer patients labeled as having myocardial injury, and robust assay-specific cutoffs that support safe use in ESC 0/1- and 0/2-hour MI rule-out/rule-in pathways.
