Article: Biological, Radiological, and Clinical Significance of Spotty Calcification in Coronary Artery Disease: A State-of-the-Art Review

This review explains “spotty calcification” in coronary artery disease as small, focal calcium deposits on imaging that likely reflect underlying microcalcification within vulnerable plaques. The authors argue that spotty calcification may have a dual role: it can be a passive marker of plaque instability and healing after rupture, but it may also actively contribute to further plaque weakening by creating mechanical stress and promoting inflammation.<br /><br />Histologic, CT, IVUS, OCT, and PET studies show that spotty calcification is closely linked to active inflammation, cholesterol crystals, thin-cap fibroatheroma, and other high-risk plaque features. Meta-analyses in the paper found that spotty calcification is more common in lesions with cholesterol crystals and in thin-cap fibroatheroma, and is strongly associated with acute coronary syndrome, especially in culprit plaques. It is also associated with impaired fractional flow reserve, suggesting functional significance beyond simple stenosis.<br /><br />The review highlights major problems in the field: definitions of spotty calcification vary widely, detection is limited by imaging resolution and blooming artifacts, and reporting is inconsistent. Because of this, the clinical value of the finding is not fully realized. The authors propose a standardized CTA definition: any calcified plaque with more than 2 consecutive pixels above 130 HU, length ≤3 mm, and maximal cross-sectional area ≤25% of the vessel lumen, with morphology described as pure or partially calcified spotty calcification.<br /><br />Clinically, spotty calcification appears more common in men and in patients with obesity, hypertension, and possibly diabetes, though the diabetes association is inconsistent. Statins and omega-3 fatty acids may reduce spotty calcification burden, but evidence is not yet strong enough to guide spotty-calcification-specific treatment.<br /><br />Overall, the review concludes that spotty calcification is a promising marker of vulnerable coronary plaque, but standardized definitions, better imaging methods, and prospective studies are needed before it can be used reliably in routine care.
