Pdf Summary
The 2026 AHA/ACC/ADA/ASN guideline introduces cardiovascular-kidney-metabolic (CKM) syndrome as a unified, staged condition linking obesity, type 2 diabetes (T2D), chronic kidney disease (CKD), and cardiovascular disease (CVD). It replaces the older obesity guideline and aims to improve prevention, detection, staging, treatment, and follow-up across the life course, including youth and adults.

Key recommendations include:
- Stage CKM disease using metabolic risk factors, kidney function (eGFR and UACR), and CVD status.
- Use the PREVENT equations to estimate 10- and 30-year risk for total CVD, ASCVD, and heart failure.
- Routinely assess social determinants of health, obesity measures, lipids, glucose, kidney function, and selected tests for pre-heart failure, MASLD, and sleep apnea.
- Use interdisciplinary care with a CKM coordination point person to reduce fragmented care.

Treatment priorities depend on stage:
- Stage 1: intensive lifestyle change and weight loss are first-line; obesity drugs or metabolic/bariatric surgery may be added.
- Stage 2: treat T2D, hypertension, hypertriglyceridemia, and CKD early; use cardioprotective antihyperglycemic agents (especially SGLT2 inhibitors and GLP-1–based therapies) and kidney-protective agents such as RAS inhibitors, SGLT2 inhibitors, finerenone, and selected GLP-1 therapies.
- Stage 3: address subclinical atherosclerosis or pre-heart failure with more intensive prevention.
- Stage 4: manage established ASCVD, heart failure, or atrial fibrillation while also treating obesity, diabetes, and CKD.

The guideline emphasizes weight management, cardioprotective diabetes therapy, kidney protection, and aggressive risk-factor control. It also addresses MASLD, venous thromboembolism, obstructive sleep apnea, pregnancy-related risk, and advanced CKD. Evidence gaps remain, especially around long-term combination therapy, advanced CKD/dialysis, and implementation of team-based care.
